Online Address Application -
 A site visit is required for all residential addresses. Your driveway must be clearly marked.   It is important that you receive written documentation of your address, therefore our policy is to not give your address to you by phone. You (or your designee) must pick up your  address form in person at our office located at 414 South Pinckney Street. Upon request, we will fax or mail a copy to you. More information
